Methodology

Hillman & Finch reticulocyte correction

corrected = retic % × Hct ÷ normal Hct; RPI = corrected ÷ maturation factor (Hct ≥40: 1.0; 30–39: 1.5; 20–29: 2.0; <20: 2.5)

Accounts for the lower red-cell mass and the earlier release of immature reticulocytes in anaemia. The maturation correction reflects Hillman's (1969) finding that circulating reticulocytes take longer to mature in more severe anaemia.

How this calculator works

Where the formula comes from

  • Hillman (1969) studied phlebotomy-induced anaemia in healthy people and showed that the time circulating reticulocytes take to mature lengthens as anaemia becomes more severe.[1]
  • He concluded that, to use the reticulocyte count as a measure of marrow production, an adjustment must be made for this longer maturation time.[1]
  • Hillman and Finch (1969) addressed the misuse of the reticulocyte count as a measure of red-cell production.[2]

Where accuracy falls (published evidence)

  • In 5,503 blood counts from children, the standard index did not agree with an age-adjusted version, and the authors concluded it is not adequate for children.[3]

Limitations

  • Automated absolute reticulocyte counts make this correction less necessary.
  • A 2020 study concluded that the reticulocyte production index was not adequate for evaluating marrow red-cell production in children.
