Methodology

TIMI UA/NSTEMI (Antman 2000)

1 point each for 7 criteria

Derived in the TIMI 11B trial; event rates rise with each point.

Who it is for
Adults with unstable angina or NSTEMI.

How this calculator works

Where the formula comes from

  • Antman and colleagues (2000) derived the TIMI score in 1,957 patients with UA/NSTEMI given unfractionated heparin in the TIMI 11B trial and validated it in three cohorts from TIMI 11B and ESSENCE.[1]

The inputs

  • Seven factors score 1 point each: age 65 or over, 3 or more CAD risk factors, known coronary stenosis of 50% or more, aspirin in the past 7 days, 2 or more anginal episodes in 24 hours, ST deviation and elevated cardiac markers.[1]
  • The outcome was death, new or recurrent MI, or severe recurrent ischaemia needing urgent revascularisation within 14 days; event rates rose from 4.7% (score 0–1) to 40.9% (6–7).[1]

Where accuracy falls (published evidence)

  • The score was derived in trial populations from the 1990s, before high-sensitivity troponin and current treatment.[1]

What guidelines say

  • The 2025 ACC/AHA acute coronary syndrome guideline describes TIMI and GRACE as validated risk scores for NSTE-ACS and notes they are risk tools, not diagnostic tests.[2]
