Methodology

AHA/ACC 2025 blood pressure categories (replacing the 2017 guideline; ESC 2024 shown for comparison)

Each number is placed in a category; the higher category decides. Severe: SBP > 180 and/or DBP > 120.

The American guideline categories are applied to each number separately and the higher one is used. The European 2024 classification is shown alongside because many countries follow it.

Who it is for
Adults aged 18 and over; seated reading taken correctly.

How this calculator works

Where the formula comes from

  • The 2025 AHA/ACC high blood pressure guideline retires and replaces the 2017 ACC/AHA guideline.[1]

The inputs

  • It keeps the categories normal (under 120 and under 80), elevated (120–129 and under 80), stage 1 (130–139 or 80–89) and stage 2 (140 or more, or 90 or more); when the two numbers fall in different categories, the higher category applies.[1]
  • The categories are based on the average of 2 or more careful readings on 2 or more occasions, and pregnancy is excluded.[1]

Worked example: how to calculate blood pressure category

Inputs

Systolic blood pressure (top number)
118 mmHg
Diastolic blood pressure (bottom number)
76 mmHg

Result

Systolic
118 mmHg
Diastolic
76 mmHg

Normal (under 120/80)

An illustration with sample values, worked out with the same method the tool above uses.

Limitations

  • Measure correctly: seated, back supported, feet flat, arm at heart level, after 5 minutes of rest and no caffeine or smoking for 30 minutes.
  • Not for children or pregnancy, which use different thresholds.
  • A category from one reading is not a diagnosis; guideline categories use the average of 2 or more readings on 2 or more occasions (AHA/ACC 2025).
